  • Email and phone number

  • A brief overview of what you’re hoping to work on in therapy

  • If you plan to use insurance and your plan and carrier (I take Kaiser, PacificSource, Moda, Regence Blue Cross Blue Shield. I can provide a superbill for out-of-network services.)

  • Your general availability for sessions (I see clients Tuesdays, Thursdays, and Fridays primarily in the mornings)

  • Anything else you’d like me to know
