If we decide to move forward with scheduling an intake, I will send you paperwork to complete online via a secure client portal.
InSURANCE & FEES
I am in network with Kaiser, Moda, PacificSource, and Blue Cross Blue Shield/Regence.
If you choose to pay out of pocket, my fee is $175 per session. I am able to provide you with a superbill to seek partial reimbursement from your insurance company using out-of-network benefits. Receiving a superbill is not a guarantee of reimbursement and I recommend calling your carrier to confirm your benefits with your carrier.
THE OFFICE & Accessability
My office is located in the heart of Montavilla across from the Academy Theater.This location is within walking distance of public transportation (bus) options. Housed on the edge of a residential neighborhood, easy street parking is available. The entrance is to the left of Vino Veritas Wine Bar.
Single stall, gender neutral restrooms are available for use down the hall from my office. Sessions are available in person at my office space or via Telehealth. I will gladly mask if requested by a client. My office is on the second floor and a staircase with 15 stairs is required for access. There are handrails on both sides of the staircase. There is no elevator available. A sofa that supports a wide variety of body types is available for clients. Information on the No Surprises Act
Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including nutrition services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including nutrition services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
